Actually, iconographically it looks most like a 4th-5th century model.

(Incidentally, this is from Lawrence Nees website - FYI Nees is reevaluating all so-called "barbarian" art. His article is forthcoming. He is an expert at 4-6th C. fibulae! Can't wait!)

Perhaps Muhlberger knows its provenance.
As for making one, I've already got my wife on it. She makes polymer clay jewelry and I think she can make a reasonable ivory substitute.

Is it just me or are the poses and the general symetry of the piece reminiscent of some of the plates from the Sutton Hoo helmet and some of the Vendell helmets?

It's not just you. Stylisitically it is more in line with 6th & 7th C.
The iconography is definitely 4th C., right down to the little cupola on christ's tomb. Based on that I would date it earlier but you're right.
could it be a 6th C. artist looking at a 4th C. diptych?
